Being a musician I understand picking up on certain nuances that the average person might miss, both verbal and non-verbal. A particular look in the eye, a twitch of the corner of the mouth, a simple shift in body position can signal a lot.
I think in card playing it might be called a 'tell'...unconscious body language that gives something away.

Bottom line as you said, only Napoleon knows Illya well enough to pick up on such tiny things... well and we the writers who torment him (them).
