The pitchfork and swamp scenes (27 and 32) are both from the Alexander Affair. Illya falling into water (43) I'm fairly sure is from the Deadly Goddess Affair. I'm still working on the others...

I'm slowly finding things:
#4 is actually Summit Five (the scene in which Beldon's shower sinks down)
#8 is Test Tube Killer
#14 is possibly Deep Six

I'm still working on 5, 12, 13 and 15

You’re right about #4. I always get their titles confused.

Deep Six is correct for 14 and 15 - I've looked through the episode, while 12 might be Prince of Darkness.
